LCIS on Core Biopsy

Question: Hi I Had a sterotatic biopsy that revealed On the pathology report that I picked up said A. Columnar cell lesion with focal microcalcifications B. Lobular carcinoma in situ. I am so confused My doctor said I have DCIS but its lobular. Treatment plan is  lumpectomy and lymph node aspiration and then 5 years of tamoxifen. She said their is a small amount of cancer cells in there. So I am unclear as to how it can be both dcis and lcis The path report does not say dcis? Thank you for your help

Answer:  If what you wrote is all that is in your report, I also don’t understand why your doctor said you have DCIS. Was there any comment or anything following the A and B diagnoses in the pathology report? Based on this, it sounds like the columnar cell lesion did not have any atypia.  Also, the treatment of LCIS on core biopsy is still controversial and the literature gives conflicting results as to the need for surgical excision so you might want to explore that more with your doctor.  I’m also a little confused as to why you need a lymph node aspiration for LCIS unless there is a clinical or radiographic abnormality in one of your nodes that is raising suspicion.   1. Hello thank you for your quick response. I met with the surgeon today and he recommended I have a the small area removed and that they check my nodes. And he had a copy of the same path report i had showing the LCIS he said it is better to get the area out so it does not turn into invasive cancer and then 1 month of radiation and 5 years of tamox. I think they just want to be safe maybe. And he did day I Have LCIS.

  2. I can understand the recommendation to do a surgical biopsy, although as I mentioned that is not uniformly agreed upon in the breast care community. However, without knowing what the result of the biopsy is, why is there a recommendation for 1 month of radiation therapy based solely on a diagnosis of LCIS on core biopsy? That is not standard.
