Getting a second opinion from SBPC
To request a second opinion from Seattle Breast Pathology Consultants, simply follow these steps:
1. Click to download the Contract for Pathology Review Form.
2. Review, complete and sign the form and send the form to SBPC via fax or mail.

3. Contact the pathology laboratory where your breast specimen was processed and read. (It is likely that they will require you to sign a release form.)
4. Have your pathology report and materials sent to SBPC. There are two ways to do this:
Choice A: Request that the pathology laboratory send the materials directly to SBPC. The advantage to this option is that labs are well-equipped to package delicate glass slides safely. The disadvantage is that—because labs process many slide review send-outs daily—there is sometimes a delay.
Choice B: Send SBPC your slides and accompanying pathology report yourself. This is typically the quicker option. Just be sure to the materials and packaged well and delivered safely via an overnight mail service such as UPS or FedEx. Also be sure to request a tracking number for your shipment.
5. The fee for review is $350. Payment must be provided at the time of submission of materials. Seattle Breast Pathology Consultants, LLC does not bill insurance companies. However, you can submit your bill to your insurance compay, many of which cover second opinions, and we will be happy to assist you with any supporting documentation you may need so this won't be an "out of pocket" expense to you. Once we have completed our consultative review of your slides and pathology report, we will promptly return the materials along with our interpretation. We will also mail or fax a copy of our consultation to any of the members of your medical team that you indicate on the contract form.
